Promissory Note

I fully understand that the non-submission of the required documents will put my enrollment status under probationary. I acknowledge that I need to submit both the physical and digital copies of the required documents for enrollment in any channel designated by PCCR. With this, failure to complete the submission within the prescribed period might invalidate my enrollment and suspension of privileges as a student of PCCR but not limited to withholding of records, non-viewing of grades and will not be allowed to re-enroll for the next semester. Completing this form means that I fully signify my intent to be enrolled in PCCR and allow the school to submit my name under the enrollment lists that will be submitted to all concerned government agencies including all affiliates and extended offices as required by all implementing rules and regulations and by law.

List of requirements should be submitted before the end of the current semester.
